[131I]HK-PEG-1 F(ab')2 is an experimental radiopharmaceutical developed for the targeted radioimmunotherapy (RIT) and imaging of gastric cancer. It consists of the F(ab')2 fragment of the murine monoclonal antibody **HK-1**, which has been modified through PEGylation (conjugation with polyethylene glycol) and labeled with the radioisotope **Iodine-131** ([131I]). The HK-1 antibody specifically targets a tumor-associated antigen (TAA) that is highly expressed on the surface of human gastric carcinoma cells. The use of the F(ab')2 fragment, rather than the full IgG molecule, allows for more rapid blood clearance and improved tumor penetration. PEGylation is employed to further optimize the pharmacokinetic profile by reducing immunogenicity and extending the circulatory half-life compared to non-modified fragments. The Iodine-131 component provides a theranostic capability, utilizing gamma emissions for radioimmunoscintigraphy and beta emissions for localized cytotoxic radiation to tumor tissues.
